Props to Keb and Caius for helping me with this. Fix is by Keb.
This is a crash likely caused by the plr_killer flag getting all jacked on ACT_POLYMORPH. It wasn't calling die_follower, therefor the person doing the following would get their master set to null. When they'd quit, the game would crash. (Keb, if you could shed a little more light on it, that would be great).

handler.c (about line 1620 in FUSS) function void extract_char()
Replace:
    if ( fPull && !IS_SET(ch->act, ACT_POLYMORPHED))
    die_follower( ch );


With:
    if ( fPull || ( IS_NPC(ch) && IS_SET(ch->act, ACT_POLYMORPHED) ) )
        die_follower( ch );



**EDIT:What about doing it the smaugfuss way?
smaugfuss handler.c void extract_char()
   if( fPull )
      die_follower( ch );


GENERAL DISCLAIMER: This bit of code has been tested to work in the Fote2.1.5 branch. It is up to you to test for swfotefuss. I'll get around to it sometime, but till then I think it should work.
