act_wiz.c, do_cedit

Locate:
   if( !str_cmp( arg2, "name" ) )
   {
      bool relocate;

      one_argument( argument, arg1 );
      if( arg1[0] == '\0' )
      {
         send_to_char( "Cannot clear name field!\n\r", ch );
         return;
      }
      if( arg1[0] != command->name[0] )
      {
         unlink_command( command );
         relocate = TRUE;
      }
      else
         relocate = FALSE;
      if( command->name )
         DISPOSE( command->name );
      command->name = str_dup( arg1 );
      if( relocate )
         add_command( command );
      send_to_char( "Done.\n\r", ch );
      return;
   }

Change to:
   if( !str_cmp( arg2, "name" ) )
   {
      bool relocate;
      CMDTYPE *checkcmd;

      one_argument( argument, arg1 );
      if( arg1[0] == '\0' )
      {
         send_to_char( "Cannot clear name field!\n\r", ch );
         return;
      }
      if( ( checkcmd = find_command( arg1 ) ) != NULL )
      {
         ch_printf( ch, "There is already a command named %s.\n\r", arg1 );
         return;
      }
      if( arg1[0] != command->name[0] )
      {
         unlink_command( command );
         relocate = TRUE;
      }
      else
         relocate = FALSE;
      if( command->name )
         DISPOSE( command->name );
      command->name = str_dup( arg1 );
      if( relocate )
         add_command( command );
      send_to_char( "Done.\n\r", ch );
      return;
   }

Obviously trying to rename a command to something that already exists is a Bad Thing(tm) since the code would just happily allow this. The old command name is unlinked, but when it tries to add the newly renamed version, it will fail becuase it already exists, thus losing the entire command from memory.
